·  Copy of MSc diploma or a certificate obtaining the Master’s degree in the area corresponding to a PhD project to be undertaken, authenticated at the institution in which the candidate studied. Do not send originals of diplomas.
·  A transcript of achievements during MSc studies (an official copy of a student's academic record), containing placement of the overall score from the studies within: top 5%, top 10%, top 25%, top 50% or bottom 50%.

·  Curriculum vitae stressing academic and scientific record

·  Documents confirming scientific achievements (reprints of publications, certificates of participation in conferences etc).

·  Medical certificate stating the ability to initiate studies in biological sciences. 

·  Confidential reference letter from a supervisor or other senior scientist at the level dr hab. or professor (or equivalent) provided directly to us on our reference form (click here to download).

Additionally, if you are not yet a enrolled on PhD studies in Biology at Jagiellonian University 

·  Polish citizens: register via ERK system (www.erk.uj.edu.pl ) and send us signed ERK form (podanie ERK). You need medical certificate concerning higher education and vocational schools (from general practicioner) attesting no contradictions for PhD Studies in the field of biology.

·  Citizens of other countries: signed authorization (click here to download) allowing us to register you in our electronic registration system (ERK), fill and sing the ERK form for you (for your convenience, we provided English translation following Polish text of the form, but you may choose to obtain a certified translation at your own expense)

Deadline for delivering medical certificates is 22nd September. Foireign students can send it by fax (+48 12 664 69 12)

·  Three photos (foregin student can deliver it after interview)

 

 Candidates will be assessed by an international panel, therefore all the above documents must be submitted in English (except for the MSc certificate, ERK form and medical certificate, which cen be either in Polish or in English).
